At Livewell Bryanston, we understand that dementia care is not a one-size-fits-all approach. Dementia is characterised by progressive cognitive decline, affecting memory, thinking, behaviour, and the ability to perform daily activities. We recently shared an article detailing our pioneering approach to dementia care titled, “The Three I’s”, which was developed by our Joint CEOs of Livewell Bryanston, Sandro Nassi and Quito Esteves. This approach to providing the highest quality of life for our residents begins with a deep understanding of these impairments and a personalised approach that respects each resident’s unique journey.

Person-Centred Approach: Personalised care plans

Personalised care lies at the heart of our dementia care philosophy. We recognise that each resident comes with their own set of preferences, memories, and life experiences, and it is our duty to honour these aspects in the care we provide. The Livewell Approach is meticulously designed to accommodate every moment of our residents’ day—from the time they wake up to their participation in meal sessions and activities.

Consider one of our residents, who had a strong preference for wearing a particular colour every day. This was more than just a habit; it became a source of comfort and stability for them. Our Occupational Therapists worked closely with the resident, initially offering three choices of clothing that included their preferred colour and two similar options. Over time, this strategy gently eased the obsession, allowing the resident to broaden their wardrobe while still feeling secure and in control. This approach not only respected the resident’s need for consistency but also empowered them to make decisions that contributed to their overall well-being.

Development of personalised care plans

The process of creating these personalised dementia care plans is both thorough and collaborative. We begin with a comprehensive assessment of the resident’s medical history, personal preferences, and life experiences. This initial assessment is critical, as it forms the foundation of the care plan, ensuring it is tailored to the individual’s specific needs.

These plans are not static; they are living documents that evolve as the resident’s condition changes. We collaborate with affiliated neurologists, psychiatrists, specialist physicians, and allied professionals to refine and update these plans. Regular multidisciplinary meetings are held to review each resident’s progress and make necessary adjustments, ensuring the care provided remains relevant and effective.

Communication: Empathy and patience in every interaction

Effective communication is vital when caring for residents with varying levels of cognitive decline. At Livewell Bryanston, we prioritise clarity, empathy, and patience in all our interactions. Our staff undergoes robust, ongoing training that equips them with the skills needed to communicate effectively, whether through verbal means or the gentle reassurance of touch.

One of our residents, who had not gotten out of bed in years at her previous care home, experienced a profound change after joining Livewell. Through ongoing reassurance and encouragement from our dedicated staff, she eventually took the brave step of getting out of bed, going through her hygiene routine with assistance, and dressing in a beautiful outfit for breakfast. The moment was so transformative that a picture of her at the breakfast table brought tears to her daughter’s eyes—seeing her mother out of her pyjamas and engaging socially for the first time in seven years was a testament to the power of compassionate care and effective communication.

Safety Measures: Balancing security with autonomy

Ensuring the safety of our residents is a top priority at Livewell Bryanston, but we also recognise the importance of promoting independence. Our approach integrates safety measures that are seamlessly woven into the daily routines of our residents, creating an environment that feels both secure and empowering.

Specific safety measures

From monitoring each resident’s manner of walking and taking corrective action to assist with mobility needs, to ensuring a secure facility with multiple layers of perimeter protection, our safety measures are designed to address the unique challenges posed by cognitive decline. The nursing team, Occupational Therapists, and carers work together to closely monitor each resident’s safety on a daily basis.

Balancing autonomy with safety

Striking the right balance between safety and autonomy is one of the most challenging aspects of dementia care. Each resident’s physical and cognitive capabilities are carefully assessed, and individualised strategies are developed to promote independence where possible. For instance, some residents may go shopping with family or participate in excursions, while others may require more structured reinforcement to balance their desires with their safety needs.

In conclusion, at Livewell Bryanston, our approach to dementia care begins with a deep understanding of cognitive impairment. By embracing personalised care, effective communication, and thoughtful safety measures, we create a nurturing environment that respects the dignity and autonomy of each resident. This commitment to individualised care ensures that our residents receive the highest quality of life, even as they navigate the challenges of cognitive decline.
